We deliver a broad range of services including Supported Independent Living (SIL), Community Access, and In-Home Support. We are currently seeking a compassionate and experienced **Service Coordinator** to join our team for a **4 month fixed term maternity leave contract**. This is a key leadership role providing oversight and support across a dedicated portfolio of participants and facilities.

**About the Role**
As a Service Coordinator, you will be responsible for the safe, compliant, and person-centred delivery of services, and for overseeing the day-to-day operations of participant homes. This role requires regular attendance at our **Strathpine office** and active involvement in managing **facilities located across South Brisbane**.

**What You’ll Be Doing**
- Oversee the daily running of participant homes across South Brisbane, ensuring a safe, well-maintained, and compliant environment
- Maintain participant documentation in line with NDIS Practice Standards and organisational policies
- Build strong working relationships with participants, families, guardians, clinicians, and external agencies
- Monitor and manage restrictive practices, ensuring alignment with Positive Behaviour Support Plans
- Provide leadership, guidance, and performance oversight to support workers
- Respond to and manage incidents, conduct follow-ups, and implement preventative strategies
- Uphold professionalism and contribute to a positive, collaborative culture within the Strathpine office

**What You’ll Bring**
- **Minimum 2 years’ experience** in a service coordination or team leadership role within disability or mental health services
- Strong knowledge of the NDIS framework, trauma-informed care, and restrictive practices
- Proven ability to manage participant supports and facility operations across multiple locations
- Excellent communication, problem-solving, and stakeholder engagement skills
- Current Driver’s Licence, NDIS Worker Screening Clearance, and Blue Card
- Willingness and availability to work from the **Strathpine office** and travel to sites in **South Brisbane** as required
