Role purpose
The Pathway to Good Health coordinator role sits within the Placement and Support Planning unit and will report to the Health, Education and Assessment Coordinator in West Metro. The coordinator will drive referrals for timely and appropriate comprehensive health assessments for eligible children in out of home care to support access to the Pathway to Good Health clinics.
The coordinator will closely with all stakeholders including child protection practitioners, agency staff, the Health and Education Coordinator and the Royal Children's Hospital Clinic Coordinator to provide information, advice and support with the referral process and problem solve barriers to attendance.
The position will assist with referrals from children and young people across North and West metropolitan areas.
