About the opportunity

We are currently looking for an experienced Care Advisor to support and grow our home care services in the (location) Region. Working closely with a supportive regional team, the position will be responsible for developing effective relationships to expand the home care services program in their local region as well as developing and maintaining a caseload of clients which will involve:
Conducting home consultations.-
- Developing and maintaining goal directed care plans.-
- Monitoring and reviewing client services and coordinating support services.-
- Managing client expenditure.What you’ll bring

To thrive in this role, you will have:
- Passion in helping people in your community, with strong relationship building, time management and budget management skills.-
- Previous case management experience as well as relevant qualifications.-
- Current Victorian Drivers Licence.-
- Strong communication and organisational skills.-
- A safe, sensible approach with a practical understanding of people’s physical limitations.-
- The ability to be flexible and adapt to meet clients’ individual needs.You will also be required to provide evidence of, or in the process of obtaining:
- National Disability Insurance Scheme (NDIS) Clearance.-
- First Aid Certificate that includes CPR.-
