We deliver a wide range of services and provide complex support across the Disability, **Out of Home Care (OoHC) and Private Aged Care sectors** in a range of settings including home care, residential, community and Specialist Disability Accommodation.

**About the opportunity**
We’re currently recruiting for a part-time or full-time permanent NDIS Support Coordinator to join our team located at our head office in Carlton.
Reporting to our Operations Manager and working with our NDIS Lead Support Coordinator, you’ll be responsible for assisting Participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan, and focus on addressing barriers and reducing complexity in the support environment, whilst also assisting Participants by ensuring they are on track to achieve their goals as articulated in their NDIS plan or Care Plans. As a Support Coordinator, you will partner with our customers and their families, and/or support networks to assist them to understand, implement and coordinate their NDIS Plan through a high quality, person-centred service which optimises the customers NDIS plan and funding. You will work closely with individuals to identify their goals and support them to access the services they need - ensuring the customer's choice, control and independence is at the heart of what you do. The Support Coordinator will also have a strong understanding of the NDIS and is required to support people who have coordination of support in their plans to access said supports effectively so that capacity is built whilst always prioritising a person’s safety and wellbeing.
**Tasks & Responsibilities**:
- Experience in coordinating responsive, individualised support to participants with psychosocial disabilityvarying in complexity.
- Demonstrated ability in establishing empowering and supportive partnerships with participants and their service providers.
- A commitment to person centred and trauma informed practice maximising the opportunities and support of participants with disabilities and their families within their local communities.
- A commitment to work within the NDIS policy and funding framework.
- Adherence to policies and programs under the direction of the Team Leader Support Coordination, and a high level of accountability.
- Demonstrated skill in researching information and developing effective and innovative support options.
- Demonstrated capacity to work independently and autonomously.
- Demonstrated ability to liaise effectively and work in partnership with all stakeholders
- Establish rapport and build strong working relationships with customers and support them to develop an understanding of their plan and plan options.
- Assist customers and (where applicable) their families, carers and nominees to understand their rights and build their confidence and abilities to make informed decisions.
- Coordinate services in Participants’ NDIS plans including the management of complex multi-disciplinary teams.
- Provide advice and consultation with Participants, their families, and other stakeholders in response to complex and challenging situations.
- Assess the funding and services which are available for the customer
- Work with the customer, and their support network, to obtain quotes and assessments to meet the elements in the plan and provide maximum value.
- Work with the customer to create a schedule of services which support needs and goals.
- Collaborate with NDIS, mainstream, and existing or potential supports in family, carers and nominees to provide customers with options for linkages, referrals and supports for customers.
- Using a strength-based approach, work with customers to increase their skills and capacity,
- Regularly review and refine plans with customers and report outcomes to the NDIA through progress reports within the specified timeframes
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments
- Researching and referring participants to relevant services when required with a focus on Recovery.
- Ensure participants are working towards achieving their goals and being able to clearly document progress in reports
