Wintringham’s Supportive Housing Project - the Audrey Rainsford Building is comprised of beautifully finished apartments centrally located in Carlton. The Supportive Housing multi-disciplinary team consists of Coordinators, Case Workers, Recreation staff and Tenancy staff, who are in place to support a staged approach to independent living and enhance the community engagement of its residents.
**Your new role**
The Case worker roles are Full Time, each working a rotating roster across Monday to Friday, including varying weekday shifts from 8:30am-9:30pm
**Your responsibilities and duties will also include**:
- Work with residents by linking and referring them into clinical services as well as social and recreational activities.
- Utilise your knowledge of harm reduction and brief intervention, trauma informed practice, conflict resolution, MARAM and Housing First principals to manage a case load of clients.
- Ensure residents obtain access to Commonwealth Home Support Program (CHSP), Home Care Packages and NDIS services by providing referrals and case management services.
- Responding to resident feedback and complaints in line with Wintringham policy and procedures.
- Utilising relationships within sectors to assist access into other services and systems.
- Working collaboratively with the other support workers and teams to ensure a collaborative team culture is maintained.
- A qualification in Social Work, Welfare, Community Services or equivalent.
- Experience in a support worker role in Intensive Case Management with specific training and/or experience in: AOD Mental Health and Family Violence or Homelessness services.
- An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
- Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
- Well-developed communications skills and the ability to with engage people from varying backgrounds.
- First Aid Certificate.
