Care coordinator for not-for-profit organisation needed in Toowoomba QLD.

Your new company
Hays has partnered with a not for profit residential, home and community services provider. Operating mainly out of NSW and expanding into South East and regional QLD with an office based in Toowoomba.

Your new role
As the care coordinator you will be responsible for contacting clients who have been referred to the organisation for assistance and completing their onboarding into the company database. You will liaise with these clients and ensure that whatever assistance and be provided is organised, rostered, communicated and essentially provided. You will communicate clearly and build rapport with these clients and provide them with whatever supporting documentation is required to complete their onboarding journey.
