The Role MQ Health is currently seeking a Care Navigator to provide the support and coordination of care for patients accessing multi-disciplinary allied health services through MQ Health. In this role, you will support patients through their end - to - end journey at MQ Health. This includes essential tasks such as actively triaging referrals, educating patients on their care plans and goals, maintaining up to date records of patient interactions and progression, whilst also ensuring necessary appointments and services are arranged for them. This role also largely supports General Practitioners (GPs) and Allied Health Professionals. You will act as the central communication point for GPs, Allied Health Professionals and patients. You will support GPs in the development and creation of care plans and facilitating essential recall and reminder processes.
