As a Care Coordinator, you will play a pivotal role in ensuring our clients receive personalised care tailored to their unique needs. You’ll work alongside healthcare professionals, manage care plans, and support both clients and caregivers.

**SKILLS**:
- Exceptional communication skills, written and verbal.
- High attention to detail
- Exceptional problem analysis and solving
- The ability to build rapport with clients, employees, and referral sources
- Knowledgeable of relevant legislation and guidelines
- Current industry knowledge (Homecare/Healthcare)
- Strong team work and collaboration skillset
- Computer literate
**DESIRABLE**:
- Formal tertiary education
- Experience exceeding 4 years in Care Coordination or Community Homecare
- Demonstrated experience using MS Suit
**ESSENTIAL**
- Valid National Police Clearance
- Current Australian Drivers Licence
- Current NDIS clearance
- Valid First Aid and CPR
