The objective of the support coordinator role is to:

- provide responsive, individualised services and supports for each person and family consistent with the relevant legislation, their NDIS plan, TLC’s support coordination policy and quality framework, and funding guidelines.
- Promote empowerment, choice and control and community inclusion of people with disabilities and their families, consistent with their NDIS plan.
- Promote a learning focussed, valuing and cooperative organisation that promotes the development and wellbeing of staff and volunteers.

Responsibilities and duties include, but are not limited to:
Person
- Understand the needs and experiences of the people we support and uphold their rights
- Empower individuals and provide clients with the opportunity to make informed decisions and choices.
- Promote dignity, privacy and confidentiality at all times.
- Work effectively with our clients’ strengths, abilities and needs, in consultation with the individual and relevant stakeholders
- Actively seek to understand the people we support and engage with community to create meaningful relationships
- Develop and maintain safe and professional relationships with clients and supported staff.
- Ensure personal data is collected and stored securely according to the individual and organizational guidelines.
- Employee/volunteer recruitment and induction
- Searching for support workers with relevant skillsets to clients taken in.
- Setting out schedule of services based on needs.
- Seeking feedback from clients to ensure that they are happy with their new support workers.
- Complaints handling
- Liaison with allied health professionals and external service providers to ensure all goals are being met or worked towards.
- Finding workers with skillsets matching the needs and goals of clients.

**Support Coordination**
- Conduct a needs assessment with the participant, based on their NDIS goals and the level of service defined in the NDIS plan.
- In conjunction with the participant and their family where appropriate, develop a support plan which contributes to the development of our client’s life skills and abilities, reflective of their goals and aspirations, clarifying the actions to be taken and outcomes to be achieved.
- Implement individual and group programs that assist clients to develop life skills, promoting community participation and integration
- Coordinate services as defined in the participant’s NDIS plan, using a strengths-based and capacity building approach. This will include informal supports, mainstream services (housing, education, health), specialist services, and NDIS funded supports.
- Empower the participant and their family by providing information about the range of mainstream and specialist services available to them, including advocacy services.
- Provide to the NDIS timely and relevant progress reports reflecting progress towards achieving plan goals, and recommendations about the appropriate type of support required in the next plan and reasons for this.
- Maintain complete case notes and other records in MyP and maintain accurate records of hours of support provided to substantiate NDIS claiming.
- Provide services and maintain written program documentation as outlined in service policy and standards guidelines, consistent with the Disability act and the NDIS Act.
- Be mindful of mandatory reporting requirements eg child safe policies, and abuse and neglect of adults and discuss with your Team Leader where a concern exists.
- Report to the Team Leader, Support Coordination on any risks that may arise in program provision that may potentially impact on the organization.
- Provide program reports and data to the Customer Care Manager, as required.
- Participate in program evaluation and development as part of the TLC’s Customer Care Team.
- Establish and maintain effective links with the NDIS, TLC programs and other provider services.
- Participate in the development of new services as required.
- Work within a person-centred framework including thinking, planning and practice
- Facilitate the development of person-centred plans
- Implement and review person-centred plans
- Monitor and record outcomes of goals on a regular basis
- Maintaining comprehensive progress notes for clients
- Ensuring client risk profiles are up to date for all assigned clients.
- Implementing and monitoring NDIS plan, assisting clients identify and achieve goals through strategy development and implementation.
- Liaise with appropriate stakeholders when required and ensure they attend meetings when required.
- Developing, implementing and reviewing goals for all assigned clients and ensuring progress is made towards those goals.
- Reviewing, Maintaining and understanding all client support plans including but not limited to, behaviour intervention support plans eating and drinking plans, epilepsy management plans, communication plans and mobility plans.
