Just Coordination of Supports (Just COS) provides _only_ Support Coordination services - Level 2 & Level 3 Coordination of Supports and Recovery Coaching Services. We are looking for a passionate, self-directed person to join our small team.

**Your new role**

Support Coordinators play a critical role in supporting NDIS participants and their support network to navigate the NDIS, understand their NDIS plans, and support them to connect with local providers and community-based programs. As a Support Coordinator, you will deliver high-quality transition support for people living with varying circumstances and levels of complexity, you will support them to build their capacity, independence and exercise their choice and control. Your caseload will be a mix of level 2, level 3 and recovery coach participants depending on the requirements of the participant.

**Your experience**

To be successful in this role, you will have a relevant qualification (Bachelor of Social Work, Psychology, Occupational Therapy or other allied health) and/or 5+ years’ experience working within the disability sector and;
- Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector
- Knowledge and understanding of health, mental health, justice, housing and other mainstream services
- Experience in reading, understanding and implementing NDIS plans
- Experience in connecting participants to services using a person centered approach
- Excellent interpersonal and communication skills, ability to build strong relationships
- Hold a current driver's license, reliable car with insurance, and willingness to travel to clients where required
- Display high level of stress resilience, emotional intelligence, resilience, reflective practice and personal accountability
- Demonstrated ability to solve problems and use initiative
- High level of organisational skills, and excellent time management skills, to work independently and manage own caseload
- Excellent IT, administrative and report writing skills
- Ability to manage budgets, negotiate and mediate, and strong ability to solve problems
- NDIS Worker Screening Check
- First Aid Certificate
- Working with Children Check

**Job Types**: Full-time, Part-time
Part-time hours: 30-38 per week

**Salary**: $45.00 - $70.00 per hour

**Experience**:
