***About the Role**:
Reporting to the Manager, Consultancy Services you will deliver Level 3 Specialist Support Coordination working proactively with NDIS participants and their families to support them in navigating, activating, interpreting and implementing their NDIS plans. To be successful in this role, you will be passionate and committed to providing a person-centred, strengths-based approach. In delivering Specialist Support Coordination to Mosaic’s growing client base, you will be confident in navigating specific high complex needs and high-level risks. You will support these clients in overcoming complex barriers to connecting with systems of support.
This work is required at all times to reflect professional boundaries, enhance positive relationships and outcomes for people living with a disability or mental health issues and work in line with the Disability Services Act 1993 and associated NSDS.
The Support Coordinator Specialist will operate within the framework of the NDIS Quality and Safeguards Commission.
**Job Requirements**:
To be considered for this exciting opportunity you will have the following:
- Experienced Support Coordinator, Social Worker or other Allied Health professional within the disability community services sector
- Bachelor of Social Work, Psychology, Occupational Therapy, or equivalent
- A passion for individualised, quality care and support
- Experience in implementing evidence-based frameworks and delivering holistic supports
- Demonstrated ability to think critically and problem solve
- An understanding of working within the NDIS environment
- Demonstrated experience working with people with complex needs and challenging situations
- Excellent communication and interpersonal skills
- Demonstrated leadership experience
**Clearances**
You will also be required to provide the following information:
- Current Working with Children Check
- Current registration with the NDIS Quality & Safeguards Commission as a Behaviour Support Practitioner
- Current Driver’s Licence
- Current First Aid certificate
- NDIS Workers Orientation Module
- NDIS Worker Screening check
- COVID-19 Vaccination Certificate
