Where this policy requirement applies, employees must provide evidence of their vaccination status including having the prescribed number of doses of an approved COVID-19 vaccine prior to the commencement of their employment.Key responsibilities of this role: - Provide general information to enquirers about the NDIS access process. Apply an end-to-end intensive case management approach, including intake and triage, to support people with disability access the NDIS. Work collaboratively and communicate effectively and sensitively with a diverse range of stakeholders including people with disability, government and non-government agencies, General Practitioners, Allied Health professionals and community organisations. Identify informal and mainstream supports that may assist a person with disability outside of the NDIS. Maintain knowledge of trends, practices, policies and legislation in relation to the NDIS to further assist a person with disability to access the scheme. Maintain accurate and up to date client case records in accordance with departmental procedures.Applications to remain current for 12 months.This work is licensed under a Creative Commons Attribution 3.0 Australia License.
