About the Role

Employment Type: Maximum Term, Full Time (6 months) – 2 Positions Available

Location: Based in Unley, with travel to Port Pirie and Murray Bridge as required

As a Child & Family Practitioner, you will play a vital role in supporting children and young people in Out of Home Care. You'll work closely with carers, government agencies, and internal teams to develop, implement, and review individualised care plans that prioritise safety, wellbeing, and long-term outcomes.

You'll use a trauma-informed, CARE-model approach to mentor carers, promote healing relationships, and support positive development in children and young people.

Key Responsibilities

  • Provide child-focused casework in partnership with carers, children, families, and stakeholders.
  • Support and supervise carers, including home visits and phone support.
  • Apply the CARE model to build resilience, skills, and trust in young people.
  • Coordinate care plans, risk assessments, and support strategies aligned with child protection standards.
  • Facilitate transitions into and between placements, ensuring thorough preparation and support.
  • Build and maintain effective relationships with stakeholders, including government departments.
  • Maintain accurate records, including case notes, carer reviews, and care plans.
  • Actively support children and young people through pathways to independence.
  • Uphold a culture of safety, inclusion, and respect in every aspect of your work.

Skills & Experience

  • Degree in Social Work, Psychology or relevant field (essential)
  • Current Driver's Licence (essential)
  • Significant experience working in Out of Home Care or child protection
  • Strong understanding of trauma, attachment, and complex behaviours in children and young people
  • Ability to deliver strength-based and culturally appropriate case management
  • High-level communication, negotiation, and documentation skills
  • Capacity to work autonomously and as part of a team, managing multiple cases and priorities
  • Understanding of permanency planning and supporting carers through change
  • Commitment to continuous improvement and reflective practice

Successful candidates will be required to clear probity checks including National Criminal History Record Check and any relevant stated based checks.
