**Key Responsibilities**:

- Develop and implement individualised support plans for clients.
- Coordinate services and resources to meet clients’ needs.
- Advocate for clients’ rights and ensure their voices are heard.
- Maintain accurate and up-to-date client records.
- Collaborate with healthcare professionals, families, and community organizations.
- Provide ongoing support and follow-up to ensure the effectiveness of services.

**Qualifications**:

- Bachelor’s degree in Social Work, Psychology, or a related field.
- 2-3 years Previous experience in a support or coordination role.
- Excellent communication and interpersonal skills.
- Strong organizational and time management abilities.
- Compassionate, patient, and empathetic nature.
- Ability to work independently and as part of a team.
