**About the Role**
The Program Coordinator / Team Leader plays a vital leadership role in managing day-to-day operations across our disability support programs. You’ll oversee staff coordination, participant support alignment, program planning, incident management, and rostering—ensuring smooth, person-centred service delivery across all sites.
You’ll work closely with the Program Director to align daily operations with Yahweh Care’s mission and values.
**Key Responsibilities**
- Lead and coordinate frontline support workers across multiple programs
- Manage daily program operations, ensuring effective service delivery
- Create and manage staff rosters to meet client needs
- Plan and monitor individualised client programs and activity schedules
- Align staff skills and availability with participant needs
- Provide leadership and coaching to support workers
- Review and submit incident reports and follow up on actions
- Maintain high standards of care and compliance with NDIS and Yahweh policies
- Collaborate with internal and external stakeholders, including families and allied health providers
**What We're Looking For**
- Minimum 3 years’ experience in a disability, aged care, or community services environment
- Prior experience in team leadership or coordination (essential)
- Certificate IV or higher in Disability, Community Services, or related field
- Strong organisational and time management skills
- Excellent communication and problem-solving abilities
- Sound knowledge of NDIS standards and compliance
- Proficient with rostering systems and incident reporting platforms
- Current First Aid & CPR Certificate
- NDIS Worker Screening Check & Orientation Module
- Working With Children Check & National Police Check
- Driver’s licence and reliable vehicle

Pay: $75,000.00 - $85,000.00 per year
