**Our Journey**:
Social Workers at Monash Children’s Hospital sit within the Monash Children’s Allied Health Team, providing specialised services to infant, paediatric and adolescent patients and their families. Social work services are provided in inpatient settings including Paediatric ICU, Children’s Cancer Centre, Neonatal Intensive Care Unit; in ambulatory care services such as the Department of Paediatric Endocrinology and Diabetes (DPED) and via state-wide services such as the Paediatric Palliative Care Program and the Victorian Forensic Paediatric Medical Service.
**A Day in the Life**:
As the Grade 2 Social Worker in the DPED team, you will be an integral team member for both endocrinology and diabetes services.
Paediatric endocrinology services are predominantly delivered in an outpatient setting within Monash Children’s Hospital. There are regular paediatric endocrinology clinics, including general endocrinology, a multidisciplinary endocrine and nutrition clinic, late effects of cancer and a paediatric metabolic bone clinic. There is also a strong liaison with obstetric, neonatal and adult services, which offers a unique opportunity within Victorian Hospitals, by providing tertiary clinical care across all age groups.
The paediatric diabetes service currently manages over 800 patients aged less than 18 years, with -approximately 170 newly diagnosed patients aged less than 19 years annually. Multidisciplinary paediatric diabetes clinics (for children and adolescents aged 15 years and younger) are held weekly, A multidisciplinary young adult diabetes service (YADS) transition clinic caters for young people with diabetes aged 15 - 25 years, and this clinic is now attended by approximately 400 young adults aged 15 - 25 years with both paediatric and adult medical, nursing/allied health support, and a strong emphasis on psychosocial support.
**What you will bring**:
- You will have a good knowledge of resources and understanding of the assessment and intervention frameworks used to guide practice including the Children’s Youth and Families Act and appropriate referral networks.
- You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and discharge planning.
- You will be skilled at supporting families adjusting to their child’s illness and able to provide grief and bereavement support as required.
- You will develop and maintain strong links with universities and your Monash Health social work colleagues and supervise students and other staff as required.
- You will be a dynamic, innovative and flexible practitioner and will receive regular supervision from highly experienced social work supervisors with opportunities for ongoing professional development.
You will be required to participate in Social Work on-call after hour’s service across the Monash Hospital sites and may be expected to work across sites within Monash Health as required.
**What you will need**:
- Minimum 2 years clinical social work experience
- A relevant social work qualification
- Eligible for AASW membership
- Experience preferably in an acute health care setting and/or with children and families
