V Connect Community Care is a growing NDIS provider that seeks to provide genuine and quality support and services to its participants. We are looking at growing our business and are looking for a new Support Coordinator.

**Key responsibilities**:

- Experience in the development of Individual Program Plans
- A clear understanding of Goal and Objective setting about identified participant needs.
- Experience in identifying, developing, and evaluating communication matters.
- Experience in dealing and communicating with people with different types of disabilities.
- Be able to work with KPI's and targets
- Converting leads into prospects
- Consulting with cold and warm leads to provide advice and solutions to participants and help them understand the NDIS process

**About you**
- Must have a minimum of 1 year experience within the NDIS industry in a similar role or have significant experience as either a NDIS Support coordinator, NDIS Local Area Coordinator, or NDIS Plan Manager.
- Self-motivated & a go-getter
- You enjoy working with participants to deliver an exceptional coordination experience
- This role is based on building relationships with industry contacts as well as participants, advocates, families, and carers looking for services
- Exceptional communication skills - you’re a natural communicator and empathetic towards our participants & their families
- Knowledge of NDIS practice standards, developing and implementation of tailored-care plans
- High-level consultation skills
- Due to the nature of the role, travelling will be required and therefore current driver’s licence is mandatory.

**You will need**:

- A valid Driver's license
- Full Australian working rights
- Valid NDIS worker screening and Working with Children check

**Job Type**: Casual

**Salary**: From $30.00 per hour
