Position Overview:
We are seeking a compassionate and organised Support Coordinator with a ‘can do’ attitude to join our team. The Coordinator will play a critical role in planning, coordinating, and managing services for individuals with disabilities, ensuring they receive the necessary support to lead fulfilling lives.

**Please be advised that this is a Part-time position, salary calculated pro rata based on 20hrs employment per week, this will be discussed at the interview.**

Key Responsibilities:

- Develop, implement, and monitor individualised service plans.
- Coordinate services and supports across various providers and agencies.
- Advocate for clients’ needs and ensure they receive appropriate services.
- Conduct regular assessments and reviews of clients’ progress.
- Maintain accurate and up-to-date records and documentation.
- Collaborate with clients, families, and multidisciplinary teams to ensure comprehensive care.

Qualifications:

- Minimum of 2 years of experience in case management or coordination or from a strong support work background who would like to excel their career.
- Strong knowledge of disability services, NDIS and community resources.
- Excellent communication, organisational, and problem-solving skills.
- Ability to work effectively both independently and as part of a team.
- Proficiency in using computer systems and software for case management.
- NDIS Worker Screening.
- First Aid & CPR Certificate or ability to acquire.
- Drivers Licence
