**About the role**
The Care Manager understands activities that contribute to the overall safety, well being, health and quality of life of an older person.
The position will ensure that care reflects individual preferences and culturally appropriate practices, while assisting clients to understand and access the level of support they require.
The role will support all clients receiving services under all Support at Home classifications. This includes self-managed clients who require a base level of care management in order for the provider to meet their obligations.
Care management involves initial and ongoing care planning, service planning and coordination, development, review and evaluation of a client’s care plan and quarterly budget, monitoring of services provided, provision of information and education about the client’s care and services, support to connect to health and other services when needed, support to undergo a Support Plan Review, if needed. Some of the main responsibilities of the role include:
- Identifying client needs, goals, preferences and existing supports
- Reviewing the client’s support plan and assisting the client to understand their approved services
- Communication with aged care workers (involved in the delivery of services) regarding the client’s needs and wellbeing
- Maintain accurate, timely, and client-centered case notes in line with Support at Home program requirements
- Document assessments, care planning, referrals, and coordination activities to support service delivery and continuity of care
- Ensure all care management practices align with Support at Home program guidelines, relevant aged care standards, and organisational policies
- Engaging in ongoing care discussions and/or case conferencing with relevant health professionals, where required
- Supporting clients (and their carer or supporter) to make informed decisions, including respecting their right to take risks, as per the supported decision-making framework
**Qualifications**
- Tertiary, ideally Bachelor level, qualification in Social Work, Allied Health, or Nursing
- Current APHRA registration would be highly regarded
**Skills & Experience**
- Demonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues.
- Clear understanding of the Aged Care Quality Standards
- Solid understanding of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program and Veterans Home Care)
- Proactive, reliable, with excellent communication skills and attention to detail
- Ability to work with others and be a participative and supportive team member
- Ability to meet compliance requirements of the role such as Current First Aid and CPR certificate, valid Victorian drivers licence with an insured and registered vehicle, ability to provide evidence of current flu and COVID-19 vaccination
