**Salary**: $55,000 - $65,000 per year
**Key Responsibilities**
Provide support, information, and advice to clients on emotional, recreational, health, housing, and other social welfare matters.
**Tasks and Responsibilities**:
- Assess clients' needs, provide direct support to people with disabilities on a day-to-day basis according to individual needs, plans, programs, and direct observation.
- Develop and implement support programs, communicate, and interact with clients, families, community agencies and other staff on day-to-day and other matters as necessary.
- Interview clients and assess the nature and extent of their difficulties.
- Monitor and report on the progress of clients, maintain client records, write notes and routine reports.
- Assess community need and resources for health, welfare, housing, employment, training and other facilities and services.
- Liaise with community groups, welfare agencies, government bodies and private businesses about community issues and promote awareness of community resources and services.
- Support families of clients
- Drive clients to and from medical appointments and other required services
**Skills & Experience**
- Relevant qualifications in the disability sector desirable
- Over two years of work experience in the industry
- An understanding of the ageing process
- A comprehensive knowledge on age as well as disability related health issues
- Understanding of and commitment to continuous quality improvements
- Excellent communication and interpersonal skills including demonstrated experience in interacting with a wide range of people.
- Ability to prioritise and manage workloads according to residents’ needs.
- Commitment to respecting values, customs, preference and spiritual beliefs of residents and their families/representatives.
**Salary**: $55,000.00 - $65,000.00 per year
