**About the role**

Assist NDIS participants in coordinating and implementing support services based on their NDIS plan using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery.

This role is **permanent part time** role (4 days/30.4 hours per week) which can be flexibly based anywhere in **NSW or ACT** with hybrid work from home arrangements.

**Key responsibilities**
- Strengthen and enhance our client's ability to coordinate supports and participate in the community
- Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency

**Skills & experience**
- Strong understanding of the NDIS, including funding and client plans
- Demonstrated experience in working with people with a disability and mental health issues
- Time management skills and ability to work within a KPI driven environment
- Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
- Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
- Established time management, problem solving and conflict management skill
- Ability to think creatively and use initiative
