**Position Overview**

We are seeking a Disability Support Worker to join our team. We are specifically looking for someone who can support a complex participant living in Ferntree Gully. Care will be provided to a young participant living with a rare genetic disoreder called Angelmans Sydnrome. The supports will be varied, but with a common goal to enhance the ability of the individual.

Assisting with appointments, daily tasks and meal times.

Personal Care (Showering and dressing)

Meaningful activities (swimming, library, cinema, zoo)

Implement strategies provided by the allied health team including, encouraging communication with communication aids/devices and strategies from speech pathologist.

**Key Responsibilities**:

- Provide support to NDIS participant, including assistance with daily living activities.
- Support participant at home with personal care
- Facilitate social and community engagement activities.
- Implement individual support plans tailored to each client's needs.
- Maintain accurate progress notes.
- Report incidents where necessary
- Collaborate with team members and healthcare professionals to ensure the best outcomes for clients.

**Qualifications**:

- Certificate 4 in Disability or equivalent qualification.
- NDIS Worker Screening Check (mandatory).
- Working With Children Check (WWC) (mandatory).
- Experience working with individuals diagnosed with Intellectual Disability, ASD, ADHD and OCD, Prader Willie Syndrome, Fragile X or Angelmans Syndrome is highly desirable.
- Strong communication and interpersonal skills.
- Empathy and a genuine desire to support individuals with disabilities.

Pay: $41.00 - $54.00 per hour
