**Pearl Home Care Canberra** is a growing small franchise within Canberra. We provide a range of home care services to aged and disability care services. Services can include but is not limited to domestic cleaning, personal care worker, transport, assistance with shopping, meal preparation, gardening, assistance with feeding, social support and allied health service.

**The role**:
The **Care Coordinator** will be responsible for the liaising with our clients to ensure the smooth running of all the support services.

You will enjoy being the link between our clients, support staff and all our external service providers to ensure that our client’s needs and goals in their care plans are achieved. You will ensure that participants have all their hours filled within a reasonable time frame.

With our **Participants and new clients**, you will be the point of contact for them on a day-to-day basis especially with their enquiries. Our nursing team will work collaboratively with you to develop and implement the participants care plan based on their available funding in accordance with budget.

With our **Support staff and all external services,** you will be responsible for allocating, organising and rostering of staff and matching them to participants. This will include, training new support workers, scheduling the services, and ensuring that care is delivered in a way that exceeds expectations whilst adhering to our policies, procedures, and WH&S requirements.

**Responsibilities include, but are not limited to**:
Provide effective guidance and mentoring to Support Workers.

Efficient allocation of appropriate Support Workers and external services to meet participants needs.
- Processing the onboarding and off boarding of staff and participants.
- Develop and maintain effective relationships with participants and support workers.
- Support the nursing team to action all customer feedback or complaints.
- Actively participate in the recruitment strategy, ongoing recruitment process and orientations.
- Promote and maintain a safe work environment in accordance with our Work Health & Safety policy.

**About YOU**:
To be successful in this role you will demonstrate strong interpersonal skills, time management skills, attention to detail and a conscientious work ethic. You will also possess:

- Certificate IV in Community Services, Ageing Support or equivalent and/or relevant experience in community care, home care or similar setting
- Must have a Sound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed Care
- Must have Significant experience and understanding in the Australian Aged Care or Disability sector
- Must be empathetic, passionate and caring
- Have highly developed organisational, administrative, and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines.
- Experience or exposure to coordinating and allocating support services to people within their homes and community.
- Experience or exposure with rostering using a digital rostering software.
- Exceptional customer service skills coupled with patience and commitment.
- The ability to build and maintain rapport with stakeholders.
- Possess a high level of computer literacy with experience using information management systems.
- Unrestricted working rights in Australia

**The rewards**:

- Hourly award rates between $33 to $40
- Option of Profit sharing
- Flexible hours with an intention of set schedule wherever possible

- Driver Licence (preferred)
- Vaccination record (required)
- Current Police check (required)

**Job Type**: Part-time
Part-time hours: 72 per week

**Salary**: $33.00 - $40.00 per hour
