**About the role**

We are seeking multiple Support Workers to join our dedicated supported-independent living (SIL) teams located in:

- _Albion Park/Calderwood, NSW_
- _Wollongong, NSW_

As a Support Worker in our team, your key duties and responsibilities will include the ability to:

- Build rapport and collaborate effectively with staff/management, participants, their family and friends to achieve a positive outcome and ensure a smooth flow of service delivery throughout the course of the participant's plan.
- Assist with liaising with participants family and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide a medium of strong communication.
- Increase participants skills with your active support and promote independence by enhancing their skills, and personal development.
- Provide regular and frequent quality and professional support in line with participants plans, needs, and wants.
- Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle.
- Implement strategies and programs to develop/enhance participant skills, identify areas of increased support, and opportunities for services to help achieve their goals.
- Be available across a 24-hour roster over a 7-day week, including morning shifts, afternoon shifts, inactive sleepover shifts and active overnight shifts. Guaranteed 8-38hrs per week.

Additional roles and responsibilities relating to this role may also include the ability to:

- be responsible for supporting participants with all aspects of their daily living, including personal care and transportation
- assist participants to engage in social, community and recreational activities in the wider community
- implementing and monitoring practices that will promote a healthy and nutritional lifestyle
- empowering participants to achieve their goals alongside our organisation
- support participants requiring a trauma-informed approach and sensitivity training to be implemented into their support services
- provide, administer or prompt to administer medications as outlined in the participant’s Medication Management Plan
- Understand and implement a participants individual Behaviour Support Plan into their support service and complete any reporting relating to behavioural incidences

**Skills & Experience**
To maintain Vital Care’s reputable name in providing extraordinary support services, our _Disability Support Workers (DSW’s) _ are required to possess the following minimum essential criteria to be considered for a position on our team:

- 12 months+ experience in supporting people living with cognitive, psychosocial disabilities
- A current certificate to provide First-Aid - _completed within the last 3 years_
- A current certificate to provide CPR - _completed within the last 12 months_
- National Standard Police Clearance Check - _completed within the last 12 months_
- A current Working with Children’s Check (WWCC) - _for paid, unsupervised role_
- A current NDIS Worker Screening Check (NDISWSC)
- A current unrestricted C-class Australian drivers license
- A completed Certificate III/IV in Disabilities - _preferred but **not essential** to the role_

**Salary**: $34.00 - $80.00 per hour
