About the role
To provide a comprehensive, high quality, Social Work service to the patients and families/carers of the Inpatient Rehabilitation Unit and Day Rehabilitation Unit. To cover Calvary palliative care and community health social work teams as per the requirements of the organisation under the guidance of Social Work Head of Department and Team Leaders.
Level 1
The Level 1 clinician is responsible and accountable for providing a professional level of service to the Inpatient Rehabilitation Unit. The Level 1 clinician will exercise professional judgement commensurate with their years of experience, as experienced is gained, the level of professional judgement increases and direct professionals supervision decreases. The Level 1 clinician will participate in quality activities and workplace education. The Level 1 Clinician may be required to provide supervision to undergraduate students on observational placements and to work experience students.
Level 2
The Level 2 clinician is responsible and accountable for providing a professional level of service to the Inpatient Rehabilitation Unit. The Level 2 clinician is a competent clinical practitioner who is able to work under mínimal direct professional supervision. The Level 2 clinician is able to exercise independent professional judgement on routine matters. They may require direct professional supervision from more senior staff members when performing novel, complex, or critical tasks. The Level 2 can assist with the supervision of university students of all levels. The Level 2 Clinician will assist in the development of policies, procedures, standards and practices. They will be expected to participate in quality improvement activities and may be required to participate in clinical research activities. The Level 2 clinician will be expected to participate in workplace education programs as required.
**Your main duties will include**:
**Criteria 1**: Qualifications in social work providing for eligibility for membership of A.A.S.W. with demonstrated ability to practice Social Work Principles of Practice.
**Criteria 2**: Experience working with people from diverse cultural backgrounds.
**Criteria 3**: Demonstrated social work experience in aged care including psychosocial assessments, counselling, and discharge planning and working in a multi-disciplinary team.
**Criteria 4**: Demonstrated advocacy skills as well as effective verbal and written communication.
**Criteria 5**: A good knowledge of relevant community resources and experience liaising with community agencies with experience in arranging supported accommodation.
**Criteria 6**: Demonstrated skills in the use of Microsoft and Outlook
**Criteria 7**: Ability to work within the Mission and Values of Calvary Health Care Kogarah
**Criteria 8**: Demonstrated commitment to ongoing professional development and workplace quality improvement.
