REQ Level 3 Social Worker - Aged Care

The Role

Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within a social work specialty area in order to provide high quality counselling, advocacy and therapeutic care to the patients, their carers and families of the facility consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

We are seeking a dynamic and experienced Level 3 Social Worker to join our dedicated team in a permanent full-time role within the inpatient Aged Care team at St George Hospital.

The Level 3 Social Worker will be part of the Aged Care team and will be the social work clinical lead in the newly established Behavioural Support Unit (BSU), located in the modern Kensington Street Building at St George Hospital. The BSU provides evidence-based clinical and behavioural support for patients with dementia, delirium, and other age-related cognitive conditions.

The Behavioural Support Unit is dedicated to supporting older people living with dementia, delirium, and other age-related cognitive conditions. Our team delivers tailored, evidence-based care that promotes comfort, independence, and quality of life. The purpose of the Behaviour Support Unit is to stabilise behaviour and safely facilitate the transition of care to an appropriate discharge destination. We offer a supportive workplace with ongoing professional development, clinical supervision, and opportunities to contribute meaningfully to a continually evolving service delivery, to meet the needs of the community.

Key responsibilities include:

  • Previous experience working in Aged Care and/or knowledge of the issues of Aging.
  • Experience providing advanced psychosocial assessment and intervention for older persons and families.
  • Supporting complex case management, discharge planning and grief and bereavement care.
  • Demonstrated expertise in NCAT processes and applications.
  • Collaborating closely with medical, nursing and allied health.
  • Offering clinical supervision and mentoring to junior social work staff and students.

We are looking for a social worker who brings clinical expertise, emotional intelligence, and leadership skills, and who is passionate about advocating for vulnerable patients and supporting families through challenging health transitions.

Selection Criteria

  • Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership of the AASW
  • Extensive post graduate clinical experience in relevant specialty area with a demonstrated commitment to ongoing clinical and professional learning
  • Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
  • Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff
  • Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature
  • Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff
  • Demonstrated ability to plan and prioritise your own and team work requirements effectively within agreed time frames
  • Demonstrated experience in providing crisis interventions and the ability to participate in the After Hours Crisis On-Call Social Work Service.
