We are seeking a compassionate **Individual Support Worker **to join our **From Hospital to Home (FH2H) **unit in South Perth, working **part-time** across a 24/7 roster.

**FH2H** **supports people with disability for up to 18 weeks; following their discharge from hospital. The unit offers a more home-like environment than hospital with all the necessary disability and social supports available, with improved access to outdoor areas and convenience for visitors so that people can plan their long-term arrangements from a more appropriate place.

Reporting to our Accommodation and Support Coordinator, you will provide empathic person-centred care for our consumers, that aims to strengthen their skills to live independently in long-term accommodation.

Your well-honed communication, assessment, and problem-solving skills are easily demonstrated when delivering culturally informed support to consumers, accessing relevant community services, as well as when working as part of an inter-disciplinary team.

- Cert III in Individual Support (Disability)
- National Police Clearance
- NDIS Worker Screening Check
- NDIS Worker Orientation Module _Quality, Safety and You_
- evidence of COVID-19 and Influenza vaccinations
- full working rights in Australia
