About the Role: Support Worker at 5-Fold Support Services

5-Fold Support Services Pty Ltd is looking for a dedicated and compassionate Support Worker to join our team in Perth, WA. This casual position involves providing exceptional care and support to our clients with disabilities, helping them to live independently and with dignity.

**What You'll Be Doing**:

- Providing personal care and support with daily living activities, assistance with meal preparation, and light housekeeping.
- Accompanying clients to appointments, social activities, and community events.
- Monitoring client wellbeing and reporting any changes or concerns.
- Advocating for clients and collaborating with allied health professionals.
- Promoting independence, choice, and control for our clients.

**What We're Looking For**:

- A Certificate III or IV in Disability, Aged Care, or a related field.
- Experience in providing person-centred support to people with disabilities, mental health conditions, or complex needs.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with clients.
- Patience, empathy, and a genuine desire to make a positive difference in the lives of others.
- A current driver's license and your own vehicle (desirable).
- A positive, solution-focused attitude and the ability to work independently as well as part of a team.

**Job Type**: Casual

Pay: $31.67 - $48.30 per hour

Expected hours: 4 - 50 per week
