**Ahanacare**is looking for an experienced and exceptional **Welfare Support Coordinator/ Intake officer** on a FULL-TIME basis to join our team at Morley, Western Australia, reporting to the Service Manager or Director.

**About Ahanacare**

We work directly with National Disability Insurance Scheme (NDIS) participants to help them live a more independent and fulfilling life. We are committed to providing our clients and their families with the support and services they require so that they can live with a sense of purpose, belonging and self-determination.

**Ahanacare is a registered NDIS Provider to deliver the following supports**:

- Development-Life Skills
- Daily Tasks/ Shared Living
- Assist-Life Stage - Transition
- Personal Activities
- Community Participation
- Support Coordination (including Specialist)
- NDIS Recovery Coach

**Role and Responsibility**
- Liaise with all members of the support/care team and provide coordinated care.
- Carry out comprehensive and accurate client assessments.
- Contribute to the development, implementation, monitoring and review of care plans to meet client’s care needs and to promote quality of life.
- Promote a consultative and collaborative approach to client care planning and quality-managed activities with external stakeholders such as welfare and community service agencies, private entities, and government bodies (including Service Australia and NDIS)
- Monitor and report on the client’s progress.
- Maintain appropriate records and documentation.
- Support and assist clients in life skills development, mental well-being and reaching individual goals in the community.
- Refer clients to appropriate agencies for additional help if necessary.
- Assist with the orientation of new staff and ongoing professional development of staff.

**Essential Skills and Qualifications**
- Current First Aid certification
- Relevant advanced diploma or higher qualification in community service or community development
- Member of ACWA is desirable
- Demonstrated knowledge and skills in assessment, treatment and evaluation
- Demonstrated ability to work effectively in a multidisciplinary team setting as well as autonomously
- Ability to work with people from diverse backgrounds with compassion and empathy and without judgement for their lifestyle choices, personal preferences and values
- Demonstrated experience and knowledge of NDIS and working within a legislative framework

**Desirable**
- A can-do attitude, enthusiasm and great communication skills
- Excellent prioritisation and organisational skills
- Proficient in a 2nd language other than English
