Umbrella Multicultural Community Care Services Inc. provides services to seniors to assist them to remain living independently in their homes. Our service focus on the individual needs of the culturally and linguistically diverse and LGBTIQ+ communities.
An opportunity has become available to join our unique and growing organisation with an immediate start. We are looking for a Multicultural Case Coordinator, working in the northern region, to support and assist the care finder team at our Olive Tree House office.
We are looking for a full-time worker, on a fixed term basis, until 30 June 2026 with a possibility of extension. Office hours are from 9am to 5.00pm. The role is subject to a 6-month probationary period.
Salary: SCHADS AWARD Level 4.1 to 4.4 with an hourly rate of $44.58 to $47.97 per hour (depending on experience) plus super
Option to salary package up to $15,990pa reducing taxable income.
Responsibilities include:
The Multicultural Case Coordinator– care finder services works collaboratively with CaLD communities and individuals and other support service agencies to provide specialist assistance and information to older people needing to access and better understand aged care services.
The main goal of this position is to provide specialist and intensive care coordination to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the community.
The position works collaboratively with the care finder services team at Umbrella to specifically reach people from CaLD communities with one or more reasons for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the community.
The ideal candidate will possess the following skills and attributes:
Essential
1. Relevant qualification in social work, health, aged care or equivalent work experience of more than two years.
2. Experienced in working with older people from diverse communities (including CaLD and LGBTIQ+), focusing on person-centred care.
3. Well-developed interpersonal and negotiation skills, including public speaking, to deliver education sessions in the community.
4. Well-developed case/client coordination skills, including case note writing, procedure development, report writing, record keeping and the ability to prioritise workload.
5. Demonstrated knowledge of the Australian aged care system, aged care standards and Charter of Aged Care Rights.
6. Proven understanding of continuous improvement practices.
7. Demonstrated administrative skills with intermediate computer literacy, including Microsoft Outlook, webinar and CRM experience.

3. Current Drivers Licence and own car
