**Your new company**

This Federal Government Department delivers government programs to support ex-service men and women and their families. They are currently seeking a Psychologist, Social Worker, Registered Nurse, Mental Health Registered Nurse or Occupational Therapist to join their Service Coordination Team in a full-time capacity on a 12 month initial contract.

Service Coordination is a specialised case management team that primarily works with contemporary families, providing support through challenging life situations. The Team primarily work with two types of clients: contemporary veterans at end-of-life to support them to access their entitlements and maximise their quality of life; and contemporary families following the suicide or service-related death of a veteran.
The program provides a primary point of contact to streamline the claims process and provide trauma-informed and client-centred support. The Team also provide support to ensure clients can access other supports through DVA, other government services, and community organisations. The program is expanding to allow them to provide support to more families and increase our capacity for families experiencing other life-challenges.

**Your new role**

The Case Manager in the Service Coordination Team is responsible for the assessment and risk management of clients referred to the team due to issues relating to mental health, psycho-social issues and suicide risk. This process involves speaking with clients, conducting risk assessments, identifying barriers to accessing help and liaising with relevant treating professionals where consent is given.

In your new role you will work closely with Service Coordination Team with clinical and non-clinical staff providing assistance to clients identified as being at risk and/or having complex needs. You will be responsible for:

- Providing short-term high quality, evidence based clinical services including intake, assessment and referral, complex case management, and case coordination to a caseload of clients with identified complex and/or multiple needs.
- Maintain clinical records according to standards, collating and analysing data from client records, and preparing reports as required.
- Liaise and form partnerships internally as well as other government departments and organisations.
- Undertake calls to clients where required to offer assistance or obtain further information, where this contact is best managed by a mental health professional.
- Link clients to appropriate mental health services and occasionally provide short-term case management intervention to link clients with necessary services for ongoing support.

**What you'll need to succeed**

To be successful in this role you will possess the following:

- You're a qualified Psychologist, Mental Health Accredited, Social Worker, Registered Nurse, Mental Health Nurse or Occupational Therapist eligible or registered with AHPRA or AASW.
- Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients.
- You must be an Australian Citizen.
