Join our dedicated Transitional Care Program team at Mount Barker, where you will provide short-term case management support for older people transitioning from hospital care.

This is an opportunity to work as part of a progressive, values-driven team that supports older clients to maximise their independence and return home safely or make informed decisions about their ongoing care.

**HOW WILL YOU CONTRIBUTE?**
- Deliver comprehensive case management and coordinate multidisciplinary care aligned to the needs of older clients.
- Provide support, guidance, and clinical oversight to junior staff and students as appropriate.
