About the Role

We're seeking passionate Locum Case Managers in Aged Care to support older Australians across the Northern and Central SA regions. These will be 6-month contracts with permanent options available. You'll work closely with clients receiving Home Care Packages, designing and coordinating support plans that empower independence and wellbeing.

You'll:

  • Conduct client assessments and reviews to identify goals and support needs
  • Develop person-centred care plans and manage Home Care Package budgets
  • Coordinate and monitor services delivered by internal and external providers
  • Ensure compliance with Aged Care Quality Standards and program guidelines
  • Provide advocacy, advice, and meaningful connection to enhance quality of life

About You

You bring:

  • A qualification in Aged Care, Community Services, Nursing, or a related discipline
  • Demonstrated experience in case management or service coordination
  • A sound understanding of Consumer Directed Care principles
  • Excellent communication, problem-solving, and relationship-building skills
  • Current driver's licence, police clearance, and relevant vaccinations
