At Just Better Care, Brisbane North we put a high emphasis on finding the "right person" to fit our organisation, and provide exceptional service to our customers. We are looking to fulfil a vacant Full Time position in our NDIS Care Management Team.
**The Role**
This position will play an integral role in ensuring person centred services and supports are delivered to customers in line with their service agreements. The NDIS Case Manager will have the opportunity to create their own team to assist with coordinating the delivery of services, alongside the Workforce Planning Officer and Community Support Professionals. The overall responsibility is to oversee quality operational services, maintaining efficiency, cost effectiveness, safety and meeting compliance procedural requirements, ultimately resulting in a positive experience for all stakeholders.
Reporting to the Operations Manager, the position will support the Just Better Care Brisbane North **six impact pillars; People, Product, Process, Collaborators, Consumers and Community** by ensuring that the operations in place are as effective and as efficient as possible.
**Responsibilities**
- Deliver excellent client care for our NDIS Participants including Care Planning, Assessment and Coordination
- Provide effective guidance, support and mentoring to Community Support Professionals
- Promote and maintain a safe work environment in accordance with our Work Health & Safety policy
- Develop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionals
- Work in collaboration with our Registered Nurse and Service Team to facilitate, develop and shape service outcomes
- Develop and maintain lasting customer relationships with NDIS customers
- Ensure compliance with all relevant legislative and industry standards is maintained
- Ensure the ongoing development of flexible and innovative services through continuous improvement.

- Demonstrated knowledge of the NDIS practice standards
- Excellent leadership skills to role model and develop staff to understand the complex behaviour supports of people with a disability within the human rights framework
- High level of computer literacy with experience using information management systems
- Enthusiastic and passionate about making a difference in the community, and in people’s lives
- Understanding and experience of; risk, complaints and incident management
- Understanding of care requirements for a clients with a disability and the ability to train staff in the implementation of care plans

**Compliance Requirements**
- Current First Aid and CPR certificate
- The right to work in Australia
- A current driver licence
- Working with Children Blue Card & NDIS Worker Screening (or willing to obtain)

**Salary**: $65,000.00 - $75,000.00 per year
