Ndis Support Coordinator

We are seeking passionate and dedicated NDIS Support Coordinators and Psychosocial Recovery Coaches to join our team. In this role, you will be responsible for assisting NDIS participants with varying abilities in achieving their NDIS plan goals, improving their daily living skills, and promoting their overall well-being.

**Key Responsibilities**
- NDIS Support Coordination:

- Assist NDIS participants in understanding and implementing their NDIS plans.
- Collaborate with participants to identify goals and supports needed to achieve them.
- Liaise with service providers and coordinate the delivery of NDIS-funded services.
- Maintain accurate records and documentation of participant progress and services provided.
- Psychosocial Recovery Coaching:

- Provide one-on-one psychosocial recovery coaching to help participants build resilience and improve mental health.
- Develop recovery plans and strategies tailored to each participant's unique needs.
- Offer emotional support, and skill-building to enhance coping and self-management.
- Monitor and evaluate the effectiveness of coaching interventions and adjust plans as needed.

**Your Profile of Excellence**
- ** Relevant Experience**: 2+ years of hands-on experience in Support Coordination and/or Psychosocial Recovery Coaching.

- Knowledge of psychosocial recovery principles and evidence-based interventions.
