Care Coordinator

**Right at Home Brisbane North and South **is currently seeking an **Euthanistic** and **Client Focused**, **Full Time Care Coordinator** to join our team.

If you’re someone who enjoys working within the Aged Care and Disability Care Sector; who’s looking to transition out of a care worker role or someone who enjoys liaising between clients and care workers, then this is the job you’ve been waiting for

The job will be located out of our **Stones Corner Office**, on a full-time basis, working **Monday to Friday**.

You’ll be working with up to 35 of our clients who live with a wide range of disabilities; the role will require you to travel to our clients’ homes and introduce them to our amazing care workers, manage their individual budgets, assisting in creating their support plans and promoting their independence.

Based out of our Sandgate office, you’re close to the Einbunpin Lagoon Park and only a 5min walk to the famous Doug’s Seafood Café. From our Sandgate office, you’ll be best equipped to assist our wonderful North Side clients.
- **Now, what do we want from you?**_
- **6+ months within the Aged and Disability Sector (preferably as care worker)**:

- **Valid QLD Driver’s Licence**:

- **First Aid & CPR Certificate**:

- **Yellow Card/NDIS Worker Screening Clearance (Paid)**:

- **Federal Police Check**:

- **High level of computer literacy**

**Salary**: $55,000.00 per year

**Benefits**:

Schedule:

- 8 hour shift
- Monday to Friday
- On call
- Overtime

Supplemental pay types:

- Performance bonus

Ability to commute/relocate:

- Stones Corner, QLD 4120: Reliably commute or planning to relocate before starting work (preferred)
