We are looking for passionate and caring individuals to provide intensive support and assistance to vulnerable older Australians to understand and access aged care services as well as advocate for those unable to access aged care services without support across Southern Sydney from the South East across to the South West.
**About the Role**:
Identifying individuals in need of aged care services through Assertive Outreach and developing relationships with them so support is accepted and actioned. Day to day this looks like:
- Promoting Catholic Healthcare’s Value proposition and service offering in the community ensuring clients and community groups have an understanding of aged care services
- Being a professional brand ambassador to the community working with client advocates, referral providers and understanding program requirements
- Engaging with specialised services and attending and presenting at community events, forums and expo’s
- Ensuring a strong understanding of client needs, requirements and potential by monitoring the client’s circumstances and maintaining relevant client records, to support appropriate referral and maintain care once assigned
- Attending regular meetings including monthly community practice meetings and other face to face, phone based, multidisciplinary meetings (travel may be required)
- Providing direct and indirect support to a client including engaging and building rapport and support to interact with My Aged Care (register, eligibility, book in assessment)
- Working with vulnerable program participants including those who are isolated, facing communication barriers including limited literacy skills and/or resistant to engage with services and providers to access care
To be successful in this role you’ll need
- Genuine passion for providing high quality care to the elderly and experience with vulnerable communities (or similar)
- Drivers license and a reliable insured vehicle
- Qualification in Community Services, Social Work or equivalent experience
- Strong communication skills and a heart for the community
- Excellent digital literacy and learning agility with new systems
