This position is a key member of the Services Team providing assessment, care planning, monitoring and referrals for all clients. An NDIS Case Manager is responsible for performing in home face to face assessments and partner with participants, their carers/families to assess current situations and create support plans that enable maximum independence and help participants achieve their goals. The role works specifically with NDIS participants and consists of but is not limited to the following tasks:

- Review intake information, engage new clients, establishing the NDIS agreement, care plan, budget and connect the client with services and supports
- Work mobile, conducting client visits as needed, practicing person centered care planning, providing independent advice and guidance to clients and their families;
- Work collaboratively with clients, their carers and representatives, ensuring their ongoing needs and goals are supported, to become more independent in their home and in the community;
- Liaise with Home Caring employees, stakeholders, partnered service providers on behalf of the client and Home Caring;
- Conduct reviews for all clients, revising their care plan, goals, services and budget expenditure as required;
- Ensure operations are conducted ethically, fairly and within the statutory legal and contractual requirements.

**Salary**: $61,838.13 - $80,000.00 per year

- 8 hour shift

**Education**:

- Diploma (preferred)

**Experience**:

- Case management: 1 year (required)
- Age care and Disability services: 1 year (preferred)

Licence/Certification:

- Driver Licence (required)
- Tertiary qualifications in allied health or related fields (preferred)

Work Authorisation:

- Australia (required)

Willingness to travel:

- 50% (preferred)
