Every day, Suncare employees are making a difference to real people and making people's lives better **About the role** The role of Care Coordinator is responsible for building, supporting, and strengthening the existing social, family and community networks while leading a team of workers to support the customer in their journey. You will be required to maintain a designated case load of mixed funded packages, including Home Care Package customers. Meeting with customers in their homes, you will gain an understanding of their needs and subsequently involve the relevant stakeholders in the assessment, planning, implementation, and review process to ensure that our customers achieve the goals and outcomes as identified through their personalised goals and support plans. In addition, you will lead, mentor, and supervise a small team of direct care staff, facilitate the recruitment and induction of employees, facilitate regular communication through team and individual meetings and provide opportunities for professional growth and development. This full-time permanent position, Monday to Friday (8.30am - 4.36pm), is a newly created role to support our growth in the Gympie and Tin Can Bay region, where you work with a supportive team of Care Coordinators to manage the region. As such you will be equipped with the technology and resources to complete your role outside of the typical office environment, with the opportunity to complete administrative tasks either at home, in the community or within a Suncare office, making this position an ideal opportunity if you are seeking more flexibility and autonomy in your role. **About You** A tertiary qualification in social/human services or related, and/or experience in individualised service planning, referral and care coordination is highly desirable though not essential.
