**Responsibilities will include**:

- Working within a person-centered, enabling environment that respects the rights and needs of each client;
- Manage case management for a variety of aged and disability clients;
- Manage Home Care Package funding administrative tasks;
- Undertake assessment and Care Planning activities;
- Refer clients to broader health providers where required;
- Provide support to access social and recreational activities;
- Observe and manage client deterioration and implement changes to care planning;
- Work under our Clinical Manager to undertake skilled nursing tasks;
- Respond to emergency situations and manage incidents.

**Essential Criteria**:

- Minimum Diploma of Nursing
- Excellent verbal and written communication skills
- Willingness to learn, develop and take on new challenges
- Current National Police Check
- Current First Aid & CPR certificates
- Current unrestricted driver’s licence
- Medication Assistance
- A reliable, well-maintained vehicle
- Comprehensive car insurance
- NDIS Worker Screening including Blue Card (or equivalent)
- Community experience, 1 year minimum (preferred)

Enrolled Nurse, Registered Nurse, OT or Physio preffered. Would consider a very experienced Coordinator with Cert IV I in Individual Care/Aged Care or equivalent.

**Job Types**: Full-time, Part-time, Permanent

Pay: $45.00 - $55.00 per hour

Expected hours: 38 per week
