Based in Strathpine, the successful applicant will work within a small multidisciplinary team inclusive of Occupational Therapy, Physiotherapy, Speech Pathology and other clinical service positions to provide Social Work and Support Coordination services to the customer, their families, and the local community

**About the Role**:
We have a rare and fantastic part time (3 days per week) opportunity for an **experienced** Social Worker to provide evidence based:

- Social Work
- Support Coordination

Northcott has a strong history of providing social work services to our community. We are heavily committed to supporting our customers in a range of different areas.

In this role you will join our multidisciplinary team of Social Workers, Speech Pathologists, Occupational Therapists and Physiotherapists who provide evidence informed and contemporary therapies for our clients. Our multidisciplinary and interprofessional approach means our therapists work together so customers can get the most out of their therapy and reach their goals successfully.

You will have a demonstrated referral partner network to draw upon and the capacity to build/strengthen relationships with other relevant organisations to facilitate both interim and ongoing support for our customers where required.

There is travel involved across our service centres. We provide company vehicles for travel but you may also be required to use your own vehicle at times.

Our centres are based in Brisbane South (Darra), Brisbane North (Strathpine), Sunshine Coast, Gold Coast (Arundel), Slacks Creek, Ipswich and Toowoomba. This role is based at our Brisbane North (Strathpine) Centre.

**Key Responsibilities**:

- Meeting discipline specific standards of practice
- Achievement of customer goals as per Goal Achievement Plan (where appropriate)
- Mentoring and support of colleagues including outcomes and recommendations
- Support of organisational culture
- Active participation in ongoing learning activities or professional development
- Effective and collaborative communication with customers, team members and others.

**Essential for this role are the following**:

- Previous experience in disability services, or other relevant community services sector;
- Social Work and/or Human Services degree or other relevant qualifications;
- Membership with the AASW;
- Demonstrated clinical competencies;
- The ability to provide staff mentoring, training and support on job related training needs;
- Excellent customer relationship management and a proven ability to manage customer service across multiple stakeholders within an organisation;
- Work collaboratively with colleagues in a multi-disciplinary services environment;
- Excellent administrative and communication skills;
- Ability to travel to all service centres across Northcott to provide support for customers;
- Availability for on call support on occasion;
- Current ‘C’ class driver’s license.
- Computer literacy in Microsoft Office Suite.
- Current working with children (blue card) and NDIS worker screening
- Provide a certificate of completion for the NDIS Worker Orientation Module.
- Be an Australian or NZ Citizen or Permanent Resident, or have a relevant visa
