**What You Will Be Doing**

Peninsula Health is seeking a Grade 1 social worker with experience working in hospital settings to join our team. This role will be part of dynamic multidisciplinary teams providing safe, personal, effective and connected care to patients across two sites. The team will work proactively and flexibly to ensure patients receive timely care in the right setting and that unnecessary admission to hospital is avoided.
Social Workers at Peninsula Health work are integral members of multi-disciplinary teams, providing quality care to patients and their families across a range of acute wards, including neurological and general medical wards. This is an opportunity to progress your Social Work career.

**What You Need**

We anticipate you will have:

- Member of or eligible for AASW membership
- Experience in a hospital or related setting.
- Demonstrated ability to work collaboratively.
- Excellent skills in assessment, counselling and discharge planning.
