**About the Role**:
Are you passionate about making a positive difference in the lives of people with disabilities? As a Team Leader, you will lead a dedicated group of professionals in providing center-based and community-focused activities. Your role will be crucial in developing and implementing person-centered plans, fostering social inclusion, and delivering meaningful activities that align with the NDIS goals and individual needs of clients.

- Lead the development, implementation, and review of person-centered plans
- Plan daily activities aligned with NDIS goals and individual needs and collaborate effectively with stakeholders and families
- Maintain accurate record-keeping, documentation and compliance requirements
- Ensure continuity of service through roster planning and recruitment
- Conduct team meetings, staff supervision, training and performance reviews
- Uphold Unisson's Values and Code of Conduct in all interactions and drive a culture of respect, diversity, and inclusion
- Manage budgets and ensure centers operate within financial limits

**A Bit About You**:
To have success in the role you will be a dynamic and empathetic leader with the ability to drive person-centered planning and service delivery. You will also have:

- Demonstrated coaching skills to provide dignified and respectful care
- Prior experience in the disability/NDIS or community services space.
- Capacity to lead person-centered planning and service delivery and the ability to translate individual goals into meaningful activities
- Experience prioritising and overseeing compliance and risk management requirements.
- Strong communication, problem-solving, and conflict resolution skills with a experience in managing staff performance and disciplinary matters professionally and effectively.
- Valid NDIS Worker Screening Check, Driver’s Licence, First Aid Certificate and Vaccination against COVID-19 (including booster)
- Willingness to undertake a National Police Check
