**Description**:
Hartley Lifecare is seeking a **Permanent Part Time Disability Support Worker** to support two young, active men in their own home. It will be an advantage for you to be physically and socially active to support a broad range of interests and community based activities.

Do you have the ability to work closely with the families of the people we support and to motivate and work alongside other team members who are committed to the integration and inclusion of people with a disability in the community? Do you have the availability to commit to the below working schedule? This role is perfect for you.

The position is **104.5 hours** per month in the following roster:
**Week 1: Fri **1130-2000 **Sat **1500-2000 **Sun** 1000-1300 **Mon** S/O **Tues** 0600-1500

**Week 2: Thurs** 1600-2200 **Sun **0700-1300 S/O **Mon** 0600-1500 **Tues** 1900-2200 S/O **Wed** 0600-0900

**Week 3**:Fri** 1130-2000 **Sat** 1500-2000 **Sun** 1030-1300 Mon S/O **Tues** 0600-1500

**Week 4: Thurs** 1600-2200 **Sun **0700-1300 S/O **Mon** 0600-1500 **Tues **1900-2200 S/O **Wed** 0600-0900
