About the role

Heartfelt Personalised Support is seeking a compassionate and dedicated Disability Support Worker to join our team in Canberra. As a Disability Support Worker, you will play a vital role in enhancing the lives of individuals living with disabilities, providing personalised care and support to help them achieve their goals and lead fulfilling lives. This full-time position offers the opportunity to make a meaningful difference in your community.

What you'll be doing

  • Assisting clients with daily living activities such as personal care, meal preparation, and household tasks
  • Facilitating community access and participation, accompanying clients to appointments, social activities, and recreational outings
  • Advocating for client needs and collaborating with other healthcare professionals to ensure holistic support
  • Implementing personalised support plans and documenting client progress
  • Maintaining a safe and inclusive environment, adhering to all relevant policies and procedures
  • Providing emotional support and empowering clients to reach their full potential
  • Can manage and handle psychosocial clients

What we're looking for

  • Demonstrated experience in providing person-centred support for individuals with a range of disabilities
  • Strong communication and interpersonal skills, with the ability to build trusting relationships with clients
  • Empathy, patience, and a genuine commitment to making a positive difference in the lives of others
  • Relevant qualifications, such as a Certificate III in Individual Support or equivalent
  • A valid driver's license and access to reliable transportation (important)
  • First aid certification and a willingness to undertake ongoing training
