About the Role
The transition pathways officer has responsibility for leading and undertaking key tasks including:
- Supporting a cluster of schools to interpret and enact departmental policies, practices and procedures relating to students exiting school to ensure students are remaining in an eligible post-school education, training, or employment option
- Provide support to schools to develop successful strategies to retain or transition 'at risk' students to a suitable education, training, or employment option;
- Develop and maintain professional knowledge of available course and employment information, particularly within their cluster communities. Ensure that they remain up to date with eligibility requirements for employment, training and education options.
- Manage and seek partnerships with providers of employment services, training programs, and employers to maximise the employment and training opportunities for secondary students who exit school before the completion of Year 12.
- Establish and maintain effective networks with school based support staff across a cluster of schools to maximise the transition outcomes of students at risk of not successfully transitioning from school before the end of Year 12
- Maintain a reporting framework and report on the progress of identified 'at risk' students to individual school principals and/or their nominee as well as reporting as required to relevant regional personnel and the Reference Group;
- Work in a network of TPOs to share information and assist with access to a wide range of services across the metropolitan region.
- Provide appropriate support, intervention and case management to young people and their families to remove barriers and assist the transition of young people into employment, training or education.
- Attend student exit meetings; and work collaboratively with school staff ensure that students exiting mainstream schooling do so with dignity.
- Work with students to develop a personalised Pathway/Transition plan to establish appropriate education, employment or training options.
When working in regulated employment an employee must have a current Working with Children Clearance (blue card) issued by Blue Card Services.
