Beata Homecare provides home care services to support the independence of clients from various cultural backgrounds in Melbourne and the Bass Coast Region.

We have a traineeship opportunity available for a Service Coordinator role, working as part of the Clinical team to assist in coordinating services for our Home Care Package consumers in the Bass Coast Region. You will gain on-the-job training while studying a course in Community Services (Case Management).

**You will be based at our office in Mount Waverely and will be expected to travel to consumer's homes in the Bass Coast region at least once a week.**

This is a full-time position, Monday to Friday.

**Role**:
Working under the direction of a Case Manager, you will be responsible for providing consumer-centred care coordination to consumers through the following:

- Organise internal and external (through contractors) support services
- Regular contact with consumers and representatives
- Undertake safety assessments
- Respond to complex/challenging situations and feedback
- Undertake administrative support including documentation, filing, data entry

**Requirements**:

- Commitment to a course in Community Services (Case Management)
- Genuine passion to help people
- Excellent written and verbal communication skills
- Outstanding time management and organisational skills
- Be able to prioritise and work through multiple tasks
- Great problem-solving skills
- Good computer skills
- Victorian Drivers Licence and reliable vehicle with insurance
- Police Check (within 3 months) or NDIS Workers Screening Check
- First Aid Certificate (within 3 years)
- CPR Certificate (within 1 year)
- Minimum 2 doses of the COVID-19 vaccination
