Beata Homecare provides home care services to support the independence of clients from various cultural backgrounds in Melbourne and the Bass Coast Region.
We are currently looking for an experienced individual to join our team as a Case Manager, working with Home Care Package clients in the South Eastern suburbs and Bass Coast region.
You will be based at our office in Braeside, Victoria.
This is a full-time position.
Role:
You will be responsible for providing consumer-centred care coordination to your consumers through the following:
- Undertake assessments
- Develop, implement and evaluate care plans
- Coordinate support services
- Engage with external stakeholders
- Manage complex and challenging situations
- Undertake administrative support
- Undertake quality assurance activities
- Manage home care package funds and budget
- Provide support to direct care workers
This role will involve travel to consumers' homes in metropolitan Melbourne and the Bass Coast Region.
**Requirements**:
- Experience in Aged Care, Disability or Community Services Sector is required
- Good computer skills
- Excellent written and verbal communication skills
- Problem-solving skills
- Proactive, Compassionate and Patient
- Victorian Drivers Licence and reliable vehicle with insurance
- Police Check (within 3 months) or NDIS Workers Screening Check
- First Aid Certificate (within 3 years)
- CPR Certificate (within 1 year)
- Full vaccination for COVID-19
