We are working with a respected aged care services organisation who are looking for a Clinical Care Manager to join their Sunshine Coast office in a community-home-based full-time permanent opportunity.
In the Clinical Care Manager, you’ll use your clinical, customer service, and aged care management skills to make an impact on your clients and their needs.
A typical day includes:
- Coordinate resident clinical care needs
- Managing care plans through clinical judgment, creativity, and sensitivity to develop care plans that are aligned to the client’s assessed needs
- Develop, monitor, reassess, and documentation STRC, HCP, CHSP care plans in consultation with clients, their families, and other allied healthcare professionals
- Work in collaboration with clients and their families manage to facilitate and develop service offerings.
- Manage and assist with incidents and complaints
You will be part of a multi-disciplinary team that work collaboratively with ongoing support from management.
**About you**

Building relationships and trust amongst your customer, you will have a genuine interest and passion towards aged care and putting the aging Australians and their families at the heart of what you do.
**You will**
- **Unrestricted AHPRA Registration, with a minimum 2 years practicing as a Registered Nurse**:
- Hold a current Police Clearance (or be willing to obtain)
- Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired
- Frontline management skills
- Current First Aid Certificate and current CPR certificate
- The right to work in Australia
- A current driver license, registration, and access to your own car with comprehensive car insurance
