This position has been created to ensure that all clients in the **Care of the Minister (Anglicare OOHC/PSP) with an identified and/or diagnosed disability** receive the appropriate and disability-specific pathways into and through the **National Disability Insurance Scheme (NDIS)**.

We seek a **Support Coordinator** for a 6-month maternity leave cover to ensure that children and young people in our Permanency Support Program (OOHC/PSP) receive the support they need through the National Disability Insurance Scheme (NDIS).

**_ As a Support Coordinator, you will:_**
- Provide disability-specific pathways into the NDIS for children and young people in the care of Anglicare.
- Collaborate with PSP Case Managers to create tailored, person-centred support plans that align with NDIA funding.
- Offer ongoing support coordination for children and young people with a disability, ensuring they have access to the services they need to thrive.

**What we're looking for**:

- Tertiary qualifications in a relevant field (desirable).
- Experience in disability services, NDIS support coordination, or case management.
- Strong communication and organisational skills.
